Located in City of Pomona, Well and Fit Adult Day Health Care Center has been providing Long Term Care Services, namely Community Based Adult Service under the coverage of State of California Medi-Cal Managed Care Program since 2002. Well and Fit Adult Day Health Care Center has established its sound reputation among community members with outstanding service quality, conscientious management and advanced equipment. We provide a structured day program that includes nursing, restorative therapies such as physical, occupational, and speech therapies, social services with supportive counseling, diverse recreational activities, meals, and door to door transportation. The program is designed for physically and/or cognitively impaired seniors and adults who wish to maintain independent living within the community. The center strictly abides by Federal and State law and regulations. No Person in the program will be discriminated against due to race, color, national origin, sex, age or disability.

Well and Fit Adult Day Health Care Center has well-designed organizational structure and most advanced facility set up. The large 12320 square foot facility allows for diverse space arrangements, including performing stage, reading area, rehabilitation area, nursing station, Karaoke room, multimedia classroom and activity rooms, for services rendered to more than one hundred participants daily. All rehabilitative and exercise equipment are selected and employed to facilitate person centered treatment and recovery plan. We introduce natural light, combined with open space concept and colorful design to create a lively environment.

Support for Family Caregivers

Due to language and culture barrier, as well as lack of information and assistance on obtaining social service benefits in a foreign country, most senior immigrants spend day time alone at home. Tied to busy work schedules, family members find it difficult to provide care needed by their loved ones, especially when they start to show signs of impaired self-care ability, memory and judgment decline or when stroke or accident happens. When constant attention and supervision are required, family caregivers tend to get overwhelmed and drained emotionally and financially.

Door to Door Transportation

We provide door to door daily pick up and drop off service. Our transportation vans are operated by highly qualified drivers who possess both First Aid and CPR certification. Their courteous service and gentle attitude will take away your worries on the road.
Well and Fit Adult Day Health Care Center is the only Community Based Adult Service center cover vast geographic service area in 3 different counties, including Los Angeles County, San Bernardino County and Riverside County.
